A implementação da gestão plena: análise a partir da eficiência técnica e social no sistema municipal de saúde de Sant’Ana do Livramento-RS

Abstract

This study aimed to analyze the implementation of Full Management, based on technical and social efficiency in the municipality of Sant'Ana do Livramento/RS, from the evolution of indicators of structure, outcome and social participation in health management in the municipality. The most decentralized municipal management qualified in the full management model of the municipal health system between 2014 and 2018 was analyzed inorder to verify if there was improvement of the technical and social efficiency of the system.The municipality joined the Full Management of the health system in 2016, which established the responsibility of the Municipal Manager for processing production contracting and payment of outpatient and hospital providers with resources of medium and high complexity. The theoretical framework presents the theoretical support of this study, based on several authors, within the proposed theme, in order to support the analysis of the collected data and is divided into Policy Concepts, Public Policy Evaluation, Full Management and Technical Efficiency. and social. The research is characterized as document analysis and data collection took place in three ways: First through the collection of secondary data directly in the DATASUS, TABNET, BI Public Portal, SISPACTO (systems that contain the municipal health records, health regions, states, and Federal District). Secondly, through the access law information to collect data regarding the appointment of health counselors. And finally, documents on holding municipal health conferences and applied content analysis to develop the results. As a result, it was possible to identify the increase of transfers from the national health fund to the municipal health fund of the municipality, but without a real increase in the existing professional structure and services, also without major changes in the result indicators. The numbers remained unchanged, except in some cases, and in the channels for social participation, parity was maintained in the municipal council and the holding of only one municipal health conference in the period analyzed. There was a need for greater planning, greater investment in improvements, evaluation by municipal management and increased channels for social participation, which are fundamental for improving the quality of public service.
