Comparação da taxa de sucesso de desmame ventilatório entre os índices integrativos de desmame e desmame convencional em pacientes em ventilação mecânica prolongada internados em UTI

Abstract

The importance of using a protocol based on the use of integrated predictive indices of weaning from mechanical ventilation (MVD) in patients admitted to the ICU, as they can increase the success rate of MVD for these patients. Objective: Compare ventilatory weaning success rates between indexes integrative and conventional. Methods: Randomized cohort study with 26 patients admitted to the ICU of Hospital Santa Casa de Caridade de Uruguaiana (HSCCU). It was carried out by comparing two groups of patients on IMV, regardless of the ventilation mode used. Participants were divided into 2 groups, one as conventional ventilatory weaning and the other as ventilatory weaning based on the use of integrative indices. Results: Two and twenty-four individuals completed the protocol in the integrative and conventional weaning groups, respectively. The time on mechanical ventilation (p=0.99) and the length of hospital stay (p=0.99) were not significant. We found no differences between the pre- and post-intervention periods for the group integrative weaning (p>0.05). However, there was lower tidal volume and work respiratory (p<0.05) and greater static compliance (p=0.02) post-intervention of the integrative weaning group when compared with the conventional group. No we found no significant value in the correlations between ventilation time mechanics and Tidal Volume, Static Compliance, Work of breathing and PaO2/FIO2. Conclusion: There is no difference between the use of predictive weaning indices in relation to conventional weaning on the success rate of ventilatory weaning and length of stay in patients admitted to the ICU.
